The context wasn't different in 1991. Ozzy got his share of criticism when NMT was released and the single bombed. I rememer an interview where he tried to position the song as a scary story and a warning (kind of like Black Sabbath the song is warning against Satan) and that he would personally "cut the nuts off" or something to that effect to anyone who would try to get near his children.
