Oh, good call. Fucking classic.

Freakiest film I've ever seen is probably Brian Yuzna's Society. I like a good weird horror and they don't come much weirder than that.

Surprised no one has mentioned Men (2022). The end scene is so weird/freaky that people started leaving the theater. I'm an avid movie goer & in all my years I've never seen people get up and leave because they couldn't take it.

Is Human Centipede not considered freaky-ass anymore? I find most all European "horror" to be freaky/bizarre.

Human Centipede 1 is not considered freaky-ass at all, compared to HC 2 and 3.

I don't like horror films so won't ever see many on this list but I did watch Mother, because based purely on the cast list I wasn't expecting what it turned into. I can't say I enjoyed it but it was definitely thought-provoking. I preferred 'Pi' though, which is hardly straightforward either.

I know that the director clearly has a message, but why be so over the top and pretentious about the whole thing? He must have gotten loads of money from the studios, since Black Swan was such a huge hit.
And Jennifer must have thought she was going to win some major awards for appearing in it, as most of his actors do win some major awards.
Actually enjoyed the film when I first saw it years ago in a theater. Definitely one of those movies that swallows you up when seen on the big screen.

But anyway . . . back to the subject at hand:

What could be freakier than a Showa era Godzilla movie? Nothing . . . 8)

It's obvious to pick Cronenberg but I can I still remember weird bits of Videodrome and The Naked Lunch after seeing them once each, 30 years ago.
