Curious, how do settlements of this nature work? Does he have to fork over the full amount in a check? Or make regular payments until its all paid off? How does that get decided?

All depends on terms of the settlement. Think of how many settlements who’ve heard about (sometimes becoming public only after someone defaults) after someone failed to make an agreed upon payment or paid part and then tried to get out of the deal, etc.
